I am fully aware of the risks and hazards connected with Gustavo Machado Punta Gorda Carvalho LLC, wellness activities. I voluntarily assume full responsibility for risks taken by me. I voluntarily assume full responsibility for any risk of property damage, personal injury, including death that may be sustained to me or to any loss or damage to any property owned by me as a result of my decision. I accept any and all costs and or treatments for myself and my family should medical treatments of any kind be needed by me or my family as a result of my/our decision. I further hereby agree to indemnify and hold harmless any associated with all activities as it was a decision I have thought through for myself and will move forth without indemnification. I, here by agree to indemnify, defend and hold harmless, Gustavo Machado Punta Gorda Carvalho LLC against any and all claims, loss, liability, damages, costs and expenses, including, but not limited to, any and all liability for damaged property and/or any and all liability for personal injury or death as a result of the activity, event or use.

I, the parent or legal guardian assume full responsibility for risks taken. I voluntarily assume full responsibility for any risks of property damage, personal injury, including death, that may be sustained to my child or children or to any loss or damage to any property owned by my child or children as a result of my permission. 

I hereby grant Gustavo Machado Punta Gorda Carvalho LLC permission to use and/or publish photographs or videos of myself or child/children in print and/or electronically and posting on social media platforms. I understand and agree that these materials will become the property of Gustavo Machado Punta Gorda Carvalho LLC, and will not be returned. I hereby authorize Gustavo Machado Punta Gorda Carvalho LLC to edit, alter, copy, exhibit, publish, or distribute the photograph or video for purposes of publicizing their programs or for any other lawful purpose. In addition, I waive my rights to any compensation arising or related to the use of the photographs for videos. I release and discharge Gustavo Machado Punta Gorda Carvalho LLC from any and all claims arising out of use of the photos or videos for any lawful purpose such as publicity, illustration, advertising, and web content. Additionally, I grant Gustavo Machado Punta Gorda Carvalho LLC, it’s instructors and employees, the irrevocable and unrestricted right to reproduce the photographs and/or video images taken by me, or members, for the use of publication, promotion, illustration, advertising, or trade, in any manner or in any medium. I hereby release Gustavo Machado Punta Gorda Carvalho LLC and it’s legal representatives for all claims and liability relating to said images or videos. Furthermore, I grant permission to use my statements that were given during discussion, interview or other communication, with or without my name, for the purpose of advertising and publicity without restrictions. I waive my right to any compensation. In the case of minors, Gustavo Machado Punta Gorda Carvalho LLC has my permission to use photo or video of my child or legal custody with all the permissions outlined above. I understand the images may be used in print, publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
